Kakuzu is definitely the most heartless character but the most twisted character would be Hidan, I mean dude he just takes pleasure in stabbing himself and watching people suffer, he's a ****ing psycho

Hidan is just a sociopath, there's nothing especially twisted about that. He can make tax management seem twisted and terrifying if he really wanted. and He's 22 !! Consider Orochi-kuzu's longevity and all the effed-up creepy shit they've done and you'll get the difference
