Do you even see your own pic? Kabuto damaged his chest and it was only external damages(wounds). Nothing was severed here. Maybe it's because of the great quantity of blood around the surface between his neck and his chest that you actually don't see it properly?

He doesn't need to activate it. It's hidden somewhere in his body. Sadly, we don't have much info about where it is exactly and how he can fit his True Form inside a human body, considering that it's times bigger.
I believe that Oro keeps his true self in a chakra state and when he wants to, he turns into his True Form.
But as I said, we have too little info on that.
