I get where you're coming from, but ninja in Naruto have WAY more durability than people in real life. Even if he could crush her skull, I doubt that she couldn't heal from it. If I remember this correctly, When a concussion makes you loose memory, your brain looses one of it's connections. When you regain your memory, you regain your brain connection. This indicates that the brain can be healed by Tsunade's regen. I'd like to discuss other factors later.

Except that if something in her head or neck snaps or cracks she can't heal it. Tell me how well this thing

I really don't know what to say about the bent neck. After all, we haven't seen Tsunade in that situation. However, I should point this out. Tsunade's regen comes from a seal, not conscious chakra flow. Minato's seal on Naruto has lasted 16 years after his death. Also, it's possible that it would heal her neck at the moment that it's severed. We've seen Kabuto reduce the damage of the ransengan this way and he's nowhere near Tsunade's league. I also doubt that he could snap her neck just like that. I mean, we've never seen someone's neck being snapped by a blow even if that person's much stronger than the other.

As for speed, it doesn't make people invincible. A is much faster than Sasuke, yet he hit him. Naruto's faster than Obito, yet he put him in tight spots. I'd be inclined to agree about speed if Guy where a ranged type, but he's CQB, and one hit from Tsunade would end him

Here's a possible counter:

She can smash the ground. This either makes Guy loose his footing or makes him unable to use his full speed against Tsunade. Guy's awesome, but Tsunade survived this long for a reason.
