i believe he will have the eternal mangekio sharingan....madara (tobi) did the same thing with his brothers eyes.

It might not be the eye it self thats the problem, It might be that optic connections between the Eyes and brain degreagate from too much use and the operation adding stem cels from a family member restores the balance. Something like that.

I don't think it would matter if itachi was almost blind, all that matter's is that his eye still works, because madara's brother was also a user of the MS and he would of been blind to sum degree. Madara still took his brother's eye's and gained EMS.
