How did old man Madara die?
Well its simple.
Think back to Nagato, when Yahiko dies, he goes on a killer rampage after, "KACHIYOSE: GEDOOOOOOOOO MAAAAZOOOOOOOOOOO!"
Madara said that without being connected to the Gedo Mazo he would die.
Being summoned away from Madara caused him to stop getting the essential chakra he needed.
If this doesn't make sense timeline wise then Kishi Screwed up!

Hmm good idea, but I think that Kishi had something else in his mind.

As far as I know, we're not sure as to how old he was when this happened but yes.
Look at Naruto versus Naruto Shipuuden. Naruto and sasuke appear to be young in the first ones but very adult in the second series even though only a few years had passed.
But back to my main point.
