Possible, I took it as Gai having taken too many hits to the head to remember. Still given the expressions on their faces I could see that. Only question is who would Madara and Gai have seen at different times? There aren't many people they could have crossed paths with that know both.

No. Madara "flew by" when he went to the battlefield with Obito, Naruto, Minato, Kakashi etc. Surely, I doubt Gai's seen him, but mweh, who knows?
