Madara could not have used Izanagi because he only possesses the ocular powers of the sage of the six paths and not the physical powers which belongs to the Senju clan (the younger son of the sage) this of course is based on Tobi's knowledge and words so unless Tobi is contradicting himself or misinformed by Madara, then it is impossible for Madara to have used izanagi against Hashirama. Now about Sasuke well one of you said that "I see no reasons why EMS should not lose his eye sight if there is still bleeding after Amaterasu. It shows that the eye can still deteriorate itself" Yes Sasuke's eye does bleed after he uses amaterasu, why? because sasuke does not have the ultimate sharingan yet, he only has his own Mangekyo sharingan, only just now in the manga did he finish implanting Itachi's eyes into his body, so we do not know the result of that yet, and thus is why his eye bleeds after amaterasu. but the ultimate Sharingan should not lose any light at all is supposed to be "eternal". Now as to how Madara surived the fight against Hashirama, well remember he said that he unlocked a new eye right before his defeat, that eye being the rinnegan which he implanted into Nagato. So i guess that answers that question, the rinnegan posses many jutsus, pain brought back the hidden leaf ninja that he killed with THAT jutsu, so Madara must have used the rinnegan to stay alive. Well those are my thoughts.
