Background:
The Rinnegan was a doujutsu originally thought to be brought out when, an Uchiha would implant Senju DNA into their bodies, but several chapters ago, light shined on more of the truth. In Chapter 671, it was revealed that the Rinnegan was activated, when Madara's Indra Chakra and Hashirama's Ashura Chakra came together, to form Hagoromo's chakra. But was that really it?
Theory:
In the newest chapter of the Manga (Ch. 686), Hagoromo said that when Ashura's, Indra's, and Kurama's energies set, his chakra/spirit would be able to come out.
So now, I'm proposing that Madara didn't just get the Rinnegan on behalf of Ashura's chakra and Indra's Chakra, but also because of having had Kurama as a summoning.
When he probably used the summoning contract or when he used the Susano'O to cover Kurama, Madara probably gained some of his energy. Later in his life, this chakra would mix with Ashura's and Indra's to bring out Hagoromo's eyes into Madara.
TL;DR:
Madara didn't just get the Rinnegan by combining Ashura's and Indra's chakra, but by also having had some of Kurama's energy when he was Madara's summoning. When all three chakra's combined he got the Rinnegan.
