How is Sasuke related to Madara?

Maybe he's his great grandfather?
Because Tsunade's grandfather is Hashirama and Tsunade's old enough to be Sasuke's mother, so timeline wise Madara would fit being Sasuke's great grandfather (thats my logic anyway)

Plus face wise they look kinda similar, and Madara's hair is basically a bigger version of Sasuke's hair..

They are obviously related in some way because their both Uchiha but you could be right. hmm

The manga stated that Madara is Obito's ancestor. If I had to guess, Madara would be his great grandfather. It is possible that Sasuke is Madara's descendant too, but I don't know.

Tsunade is too old to be his mother, she would have been close to 40 when he was born.

I don't think Sasuke is directly related because Fugaku was the clan head, and I don't think the Uchiha would allow him to be head if he shared the same blood.

Something interesting to note is that Obito referred to Madara as 'ore no gosenzo' meaning 'my honourable forefather' suggesting there could be a direct link.
