he got the rinnegan when he was old right before dying, then he used the gedo to stay alive created zetsus (he thought he had created zetsus) and sent them to give his eyes to baby nagato making nagato think he was born with them.

good question..! i guess he awakened rinnegan even if the eye was blind since rinnegan depends on ashura and indra's chakra(senju + uchiha) rather than eye..! the reason he said to obito that despite having both chakras he failed to awaken rinnegan even when obito only had MS..! thus proving that neither EMS nor healthy eye is needed for rinnegan..! it's two chakras + time and skill..!

The rinnegan is a different dojutsu from the sharingan when madara awakened it he regained his vision in the right eye, but he still lost that sharingan.

As for giving nagato the rinnegan, he probably used zetsu to implant the rinnegan into him, how nagato did not know the eye didn't belonged to him I will never know.
