Second time, he was in the cave and Obito most likely buried it somewhere but we know he can't have destroyed it since Kabuto found it with BZ's help
Third time, he was a Kaguya Balloon
Final time, a bunch of people have said he was blind for no reason, so the Rinnegan might not exist any more. Chances are Kakashi destroyed it while Naruto and Sasuke were duking it out.
