Hiruzen saying "not again" as Kurama readied a TBB could imply he already dealt with Kuramas TBB atleast once before during the battle. Again Hiruzen was not in his prime condition which would explain why he collapsed. That is always a possibility. Hiruzen doesnt get enough credit and receives too much hate. If he truly knows all Konoha jutsu then he would be able to handle a TBB. We seen an Edo Old Hiruzen defend against the Buddha statue that even the Kage and rest of the alliance there could not collectively.
