Sick, nearly chakraless and on-death's-door Itachi used V3 Susano'o to protect himself from Kirin, reactivated it and later kept up a V4, the strongest and most taxing version after PS. Sasuke stated that his whole body was aching when he was also low on chakra just after using ribcage, imagine what Itachi was going through in his final moments.

Itachi has more than enough chakra and stamina to sustain PS, especially with the fact that EMS seems to remove the immense cost on the latter, since Sasuke never even showed a sign of fatigue after basically having it on 100% uptime.

Yeah, because intended to die in that battle so he was being totally reckless.

Unless he's aiming for death, he can't use something like PS. In a normal battle, Itachi needed a full night's sleep after using MS for a total of 7-8 seconds straight. He couldn't even keep 3T active after that.
