Itachi wins via Tsukuyomi, it's going to be hard to land it though, so i'll say high difficulty in the end.

Need to restrict Totsuka and Yata to prevent that argument from being made. Even without those, Itachi would be favored especially given his superior tactical knowledge, but with those he'd never really be in any danger.

--kisame gets put under tsukuyomi eventually, especially If he resorts to fighting him in CQC, given that itachis speed and reflexes are above kisames, as once that happens, kisame is done for

--kisames not countering totsuka either. It's just a GG, not to mention that kisame isn't fast enough to dance around susnaoo, as V4 susanoo cutting him in half is also plausible

--kisame admitting inferiority to itachi in canon just makes things worse

Itachi wins mid high. Truth is Amaterasu and Tsukuyomi are landing before Kisame makes any of his bigger moves.

--If Kisame spits out a large body of water (like water dome) he can remain at a distance and Itachi won't be able to reach him with Totsuka or Susanoo

In kisame's own environment? eh He'd still put up quite a fight, not like he Itachi can keep his eyes open long underwater anyways U_U

B didn't have a problem, but like I said to beans, not seeing how it being underwater matters when chakra sensing is his only way to determine Itachi's position, and Jiraiya has stated that chakra sensing isn't enough to let you fight someone blind, effectively anyway.

He has sunglasses, not goggles.

With his body? Yeah. With Susanoo? Meh. Then there's always Genjutsu. His main issue underwater is mobility, but he doesn't need to be mobile when he has Susano. Kisame's still at a bigger disadvantage though. Human Path was blocking Jiraiya's attacks without eyesight, something that Jiraiya said isn't possible with only chakra sensing.

What exactly does "fighting effectively" imply? If Kisame has already formed the water dome, is at mid to long range, and knows Itachi's position, there is literally no threat to him. Itachi won't be able to close the gap between them quickly and considering Samehada reacted to V1 Bee from close range I would think Magatamas can be dodged by fused Kisame/Samehada as well, especially if used from long range and slowed down by water pressure.

"Fighting effectively" in the sense that Jiraiya was talking about was how the pains quickly reacted and dodged Jiraiya's multiple assaults, it's a totally different situation with this battle and basing Itachi's victory off Jiraiya's statement isn't an accurate way to determine the winner anyway.

Itachi mid-diff. If Itachi gets him with Tsukuyomi or Amaterasu he's probably out, and even before then Kisame has to watch out for Itachi's 3T genjutsu. If Itachi gets pressured into Susano'o, and that's honestly a big if, Kisame can't do much to his Susano'o. Itachi doesn't really have any defense against the water dome, though, so he'll have to put up Susano'o quickly. Still, I don't see Kisame winning this.
