Why small katsuyus heals tsunade-sama? Not the very large one?
Effortless!!!

1) She asked for Katsuyu to focus on other kages.
2) Katsuyu was focusing on healing + keeping his summoning with the little chakra that Tsunade was providing.

So Katsuyu was just doing enough to keep her alive.
