Pre torture:
As a child, Kaneki had all the same features he has now as an adult with black hair. As a normal university student, Kaneki was a scrawny young man with little to no athletic background who preferred to read books. In his free time, he wore casual clothes. He stands at 5'5.
Post torture:
After being tortured by Yamori, his appearance changed drastically. His hair became completely white, and his nails were blackened. He began to favor monochrome clothing, and wore a form-fitting black bodysuit with cutouts when going into battle. His other most worn set of clothes is a black shirt with ripped white shorts. Kaneki very commonly wears a mask that takes inspiration from an eyepatch, covering most of his face and wrapping back around his head. The eyepatch that goes over his right eye is made of a fabric that works like one-way glass in that the wearer can see out from it, but not the other way. Even with the mask on, Kaneki's eyesight and peripheral vision are just as good as someones not wearing a mask.

Personality
Pre torture:
Before being tortured, Kaneki was a quite shy and well put together boy who loved reading and spending time with the few friends he had. He also was very well mannered, and had visible success in his academics. Kaneki was a pacifist, and because of this decided that he would not have anything to do with the path of becoming a shinobi. He always believed that it was better to be the one blamed than let someone else be. He attended a school that was held as separate as possible from all of the things pertaining to the ninja world.
Post torture:
After being tortured, Kaneki became cold and harsh, resorting to violence and fear tactics if necessary - he no longer embraced or believed in pacifism. He believed that unnecessary kindness was often weak, and would not allow anyone to use him though those kinds of means, or any at all. One of the only parts of his personality that transferred, unchanged, from his pre to post tortured selves was his love of literature. The other was his loyalty to his friends, even if that meant completely cutting ties with them to protect them.

History
Early ChildhoodKaneki was born from a civilian woman and Uchiha man in Konohagakure. Kaneki's mother wanted to raise him as a normal civilian, and hoped that he would become a politician. Kaneki's husband strongly disapproved of this, and wanted to raise him as a powerful shinobi who's name would be known in every corner of the world. The two reached an agreement, saying that they would have the boy to study medical ninjutsu once he became old enough. The father still planned to lead his son down the path of a shinobi, but could not argue that having knowledge in the fields of medical ninjutsu would be helpful in combat. The three lived together on the outskirts of Konoha for several years. Once Kaneki had turned 7, his father tried to beat him in order to activate his sharingan. He was drunk, but the effects of the alcohol only made him do what he would have done sooner or later. Horrified, the mother came home to Kaneki screaming and crying as he lay on the floor. Seeing her come home, the father began to scream at her for being against their son becoming a shinobi. The father left behind a kunai for Kaneki to keep, telling him that if he wished to ever face the reality of this world, and that avoiding conflict and war was impossible, he should take that up and find him in the Uchiha neighborhood. Due to Kaneki's natural belief that "it's better to be hurt than hurt people" his sharingan had not activated. The condition for the activation of a sharingan is "facing incredible fear or trauma." He felt to trauma, just sadness. He would rather be hurt than his father feel pain because his own flesh and blood was not allowed to become a shinobi.
Late Childhood
Kaneki and his mother had been separated from her husband for 2 years, during which he had moved out of the village. He attended a school which made sure to separate anything related to the ninja world from it, besides medical ninjutsu. His mother decided that she would not allow him to attend classes pertaining to medical ninjutsu, she had come to believe that chakra corrupted the mind. People at school began to make fun of him for always reading and sitting alone in the corner as medical ninjutsu was explained and taught. All but one person, a boy named Hideyoshi treated him like an outcast. Hide and Kaneki became best friends over time, as the latter was hesitant about whether Hide was serious or not. After 2 more years, two shinobi knocked on his door to tell him that his mother had collapsed and died the night before at work. Kaneki was horrified, and he hardly heard the two men ask him if he had any living relatives. Kaneki refused to say his father, so told them he had an aunt.
Teenage/Adult Years
Kaneki lived with his aunt and cousin for 7 years since his mother's death, and he was now 18. He had been facing depression for all 7 of those long years. Kaneki was still friends with Hide, and recently had picked up a job as a waiter at a coffee shop, where he met a few more people. He met a girl named Rize who he accompanied on a date later that day. They shared many similarities, and enjoyed themselves. As Kaneki walked her home that night, he was surprised to find out that she was actually a wanted serial murderer. She attempted to eat him after toying with him for a while. Unlucky for her, a mass of several steel beams crushed her before she could get a fatal wound in on Kaneki. As the mass of steel hit her, she placed a genjutsu upon Kaneki without him knowing which would only activate in the face of death. Weeks later, a co-worker of his there, an Uchiha girl named Touka told him that his father had apparently been looking for him for multiple years now. She promised not to say anything to him, but it took quite a bit of convincing and her teasing him. Returning home, his verbally abusive aunt and cousin greeted him angrily. After apologizing to them both and offering to go get them washed right then, which he was yelled at for being "fresh" and "wise", he went to his room. Inside stood his father. He wove a seal, causing the door to slam shut with a gust of wind, and a rope of wind to wrap around him. He held up a photo of Kaneki with Touka from outside of the shop that day, saying that cafe was a popular place for him to go to.
Torture:
Kaneki's father kidnapped him, bringing him who knows where, as he was blind-folded. After being asked how developed his sharingan was, and Kaneki responding essentially saying "Not at all," he was knocked out with a punch. When he woke up, he was chained to a chair in a large open room shaped like a cylinder. A man stood before him that he had never seen before, and he was talking on a cell phone. As he closed it, he said to Kaneki that his name was Yamori, and that he had been hired by his own father to introduce his son to death. He explained that his father wanted Kaneki's eyes so that he could implant them into his other son's head, and give him the Eternal Mongekyo Sharingan. Kaneki did not even know he had a brother, which left him even more shocked. Yamori began to torture him in unthinkable ways, like cutting or crushing off all of his toes and fingers, then having
them re-grow by using highly advanced medical ninjutsu, only to re-crush and cut them off again, before healing them. He also inserted a centipede into Kaneki's ear at one point, which he let crawl about in his body. He even attacked Kaneki psychologically, by trying to make him choose out of two people who to kill, then rephrasing it by saying "Which do you want to save?" After this and more, Yamori said that he gave up, and was instead of all of this hard work was going to kill him. In this moment, the genjutsu placed upon him by Rize activated. All time slowed, and Kaneki was sitting, chained to the same chair as before in a completely white room with Rize standing before him. She began talking to him, taunting him that he was going to die because of his stupid ideals and lack of power. She came back to his idea that "it is better to be hurt than to hurt" numerous times, pointing out how weak that was. She then told him that his mother also died because she was weak. She had read a report that proved she covered for her co-workers who wanted to go out every night, and overworked herself to death. She then went on about how Hide, Touka, and others would die eventually too because of stupid ideals. Though they didn't have those ideas, it still created major psychological distress in Kaneki, causing him to change. The genjutsu ended as he broke out of the chair in the genjutsu and in real life, his hair having changed to a snow white color due to a condition called "Marie Antoinette Syndrome" which occurs when someone experiences extreme stress and panic. His nails all had been turned black due to the constant removal of the fingers and re-growth of them, and his entire personality had cracked and fallen apart, giving way to a much darker and crueler version of himself. His eyes turned red, with two tomoes in each. Kaneki fought a battle, completely unexperienced in any and all forms of combat, against Yamori. Eventually Kaneki killed him and left the building.

Kaneki headed toward the clan's isolated edge of the village, and entered his father's home after forcing a patrol of the Uchiha Police to give up his location, as well as their ninja pouches and a sword. He entered his father's house and pulled out a kunai. His father, seeing him alive, attacked Kaneki and reached for his eyes. Kaneki attempted to kill him, but failed as his father fled. Kaneki chased him all the way to Amegakure over the course of several days. When he arrived there, he was angered to find that he had lost his father's trail. Throwing his headband aside, he emptied out the ninja pouches he stole, and saw a mask of sorts drop to the grass. He picked it up and examined it. Approving of it, he strapped it on and cracked his knuckles, a habit he picked up from Yamori. Kaneki decided to train in Amegakure to hone his skills as he kept tabs on Konohagakure from a far.

Kaneki's mask, when the mouth is zipped up, can filter out toxins and other bad things in the air that is free form. The covered eye from the eyepatch is also protected from bright lights which are free form.
