He only possesses Raiton, Suiton and Doton. And it's not an impossible feat to have that many natures. Most jounins have two.

He only has fire in the anime. He has never used it in the manga.

And there's nothing to really answer. He just has 3. Hiruzen apparently had all 5. I'm sure it has to do with training along with his ability to copy handsigns with the sharingan.

simple, lots of practice mixed with being a prodigy. you can learn any kind of nature but you are only born with an affinity for one

I believe he has learned them with the help of his sharingan. I guess after years of copying he learned those types, he's called the copycat ninja for a reason.

Also this could be why all his other jutsus aren't really powerful

Also with earth he only goes under or manipulates the water and the gravel to make the mud wall which makes it an earth style. There's a huge difference between being able to create and manipulate

Creating earth has only been done by Hiruzen, to my knowledge, when he did it fighting Orochimaru. But no, Kakashi has never "created" it.
