He gets physical and mental damage, but no damage of vision whatsoever.
Sasuke and Itachi are Uchihas, they can use it freely, it doesn't drain their chakra as it does with Kakashi, the reason why Kakashi is damaged.

Pain to the body is all related to the chakra drain.

Yes, his vision does get damaged in that eye. That is undisputable since he stats that himself. Just because he can't make his own eyes bleed doesn't mean he's not doing damage. It's just as said, he minimises that by having a normal eye and simply doesn't get that same reaction as he's completely drained after two usages. Read back to what he said to Itachi, how could he not know if his Sharigan wasn't suffereing simlar eyesight damage.
