Itachi could never be killed, Kishi didn't know how. He always had to die from disease or kill himself off. Getting killed by anyone else would be too unbelievable.
Kaguya was afraid of Itachi all the time.
She knew Itachi can easily overpower Mugen Tsukoyomi with his own moon dojutsu, so she waited for him to die. What she doesn't know is: It is all part of Itachi's plan, the Aizen of Naruto.
