Kaguya was in one of the sealing tools and when the chakra from it was absorbed she (and the G&S probably) was too. When Madara became the full juubi jin Kaguya was inside him and black zetsu triggered her awakening and reversed the situation (madara absorbed)

Madara spat out the tools its likely Hashirama will use them and Madara. Yup I think Madz may some how return and Hashi, Madara, Naruto and Sasuke (the reincarnation of her children/grandchildren) will defeat her in a parallel way the Sage, his bro and children did.
