If the person is kabuto, i think it might be possible, that he gives his heart sasuke.

Imagine, he could replace sasukes destroyed heart with his own.

Since he was sasoris spy, he should be able to place his heart into someone else, like sasori did with his puppet body.

Kabuto could say sth like that:

"Thank you, Itachi@ You just showed myself, who i am... I'M KABUTO YAKUSHI, THE ONE THAT SAVES YOUR BROTHER, AND CHANGES THIS WAR! This is the least, I can do!"

I dont think Kabuto is going to come back from one of the most mind fuking techniques ever used, walk miles and then to simply kill himself. That doesnt make sense. He will use his amazing medical ninjutsu skills.
