So judging from the last chapter's title and from what kabuto said when he was a spy for the stone village "If I didn't make that mistake I would of been asleep by now" (something like that).

His mistake was leaving the orphanage to join root and become a ninja. He was already getting acknowledged as a great medical nin, yet he decided to join root. Anyway I really hope this was not his mistake itachi was talking about because this.. is.. lame.
