He was caught off guard not because of zetsu's speed, but by madara's individual arrogance. He believe zetsu was his will. Maddy is not gonna let sasuke get him off guard. You're assuming a decapitation will kill Madara. He stated that he was immortal. That= He can't die regardless of a head shot. He'll just regenerate like it's nothing.
