With all the speculation of there being a slug sage mode, I'm going to say for my purpose that it exists. Do you think tsunade doesn't like to use it because much like Jman she doesn't like the way it changes her appearance, so she won't use it. I mean she uses that Justu to make her look younger do you think that she very well could not want to use it solely for the purpose of the looks. Like I said this is assuming that slug sage mode is real since it has yet to be shown or even talked about in the manga.

I doubt Tsunade.gives a shit about looks when against Madara. There's no she would hold back a Sage Mode against Madara.

Slug Sage seems far more plausible to tie into the story than Hawk or Monkey. Plus the place that was mentioned for the power is damp forest... slugs like damp.

Let me guess ->
The user have to wait 50 years to get into slug sage mode since slugs are too slow.
The user would become extremely slow since its SLUG sage mode .
The user would excrete a lot of smelly acids from its body, making him/her sluggish.

I couldn't tell you since we have no idea lol, I wouldmassumemthat it would have some of the same benefits of the other sage modes however.
