I would think through an exam or by selection. However as we saw in Road to Ninja it seems that one can be recommended for the position to then be picked by the Hokage.

A lot of Naruto fans have the wrong info, the Special Jonin which are named "Tokubetsu Jonin" are just Chunin who have special skills in particular area that made them a Jonin, therefore weaker than true Jonin who is all round skills are Jonin levels, Just like Ibiki Morino his skills are Chunin but he is a jonin-level interrogation expert therefore they promoted him to Tokubetsu Jonin.

He should do couple of missions and then be promoted, He could actually be a Jonin, if he increases his taijutsu skills that is all he has to do. Poor Rock Lee will never do a ninjutsu. After the war a lot of people will be promoted including Shikamaru in my opinion all this could happen in Naruto Akkipuden.
