She is his age but she naturally does not look her age because of her jutsu. However, the Tsunade we know even though she is using some form of high level transformation jutsu you would never tell or feel like she is old, she has some weird unique transformation, it doesn't even disappear when she gets hurt, only when chakra is gone. So there is no way he could see her naked looking old even if this happened out of prime. But as you said it happened when she was young.
