Now before you start posting it was AIDS or tuberculosis, lemme explain what I believe he had and how it was strongly hinted in the manga. I believe that he died from the disease known as Acquired Immune Deficiency Syndrome (let's just call it AIDS for short).
AIDS is a disease that affects your immune system and is a result of having the Human Immunodeficiency Virus (we'll call it HIV). HIV is primarily transmitted through unprotected ***, from mother to child during pregnancy or contaminated blood transfusions.
Now you might be wondering how Itachi got this disease but don't worry I have the answer to that as well. We all know that Itachi was the embodiment of beauty and luscious hair. As seen here
It was also confirmed that Itachi had a lover who he "killed" confirmed here
"But Itachi has the perfect counter to AIDS and he would never take such a risk". While this is true there is one way he could have had raw ***. Even though Itachi always stays protected with his all mighty Yata Mirror/Condom there was still one way to get through. Shisui's Kotoamatsukami.
Shisui was getting tired of using condoms and started to suspect that Itachi was cheating, so he used his KA in order to have raw *** with Itachi and to make their love stronger. But how did Shisui contract HIV?

, who was known for experimenting on bodies and being the NV version of Hitler, noticed that Hiruzen was very talented and wanted to see if he could survive long with AIDS so he did some experiments with him in disguise as "check ups". Hiruzen was the equivalent of Itachi in looks for that generation and was known to be a player which gave Tobirama even more reason to experiment so that if he did last long Hiruzen could spread it around enough hoping that one day Itachi will get it. As a result, Godruzen was born and ended up living with AIDS for more than half a century outliving Tobirama himself and unfortunately he couldn't see his dreams come true. So you see my children, Tobirama was the one who plotted Itachi's death in fear of him.
