Probably an arranged marriage. Other manga use the term "lovers" as well, w/o it meaning anything like a casual sexual relationship. In Sailor Moon, 12 year old girls referred to their boyfriends as lovers, but they had no such relations with them. Also, Itachi had to train, be a double spy, went on missions on his own, read about the Uchiha history, was an Anbu captain and had to arrange those missions, take care of Sasuke, etc. He's was a really busy guy and I doubt he even had time for a "lover". Fugaku probably just picked some girl out for him and told him that's who he would end up with in the future.
