Why did Itachi have bags under his eyes? He had them since he was young? Was it to show no one is perfect?

It couldn't be because he was ill, there were other ill characters in the series.

He had them slightly as a child, but they got larger as he grew older. I doubt they have anything to do with his illness, just facial features.

Itachi has tear-troughs under his eyes because of the stress of defending the 'world' from 'evil' harm, pain & other things.
Itachi has tear-troughs so he most likely cried a lot on the inside (depression) or when he was alone he cried a lot on the outside.
the weight of the Konoha 'world' & the Uchiha was on his shoulders. He was torn between them forever. He had the reasoning of Hokage at age 7 , traumatized at age 4, he obviously been thinking about protecting his 'home' most of his life. A duty he took upon himself.

The lines on Itachi's face make gives him an almost beak-like appearance. And being based on Tengu somewhat explains the bird motif Itachi has.
