I think it would look different. Probably wouldn't have straight tomoes like Sasuke, and it mostly be centered on Itachis mangekyo design with Sasukes complementing it, rather than the other way around

I think if itachi had taken someone's eyes and was still alive he'll be up to Obito's rinnegan but then Sasuke would have kill him all over again and the whole the storyline will be messed up but that's the unexplained side of my theory

It would probably be different. At the least, the colors would be inverted compared to Sasuke's, since Sasuke's Mangekyo is a red pattern on black, as opposed to a black pattern on red.

I would say that it'd be different. Sasuke had a straight tomoe, where as Itachi didn't. I assume that Itachi's EMS would keep that look, going with a more curved designed while gaining the dominant black color of Sasuke's.
