Itachi and Shikamaru are both ranked 5 in intelligence. Even without prior knowledge of Shikamaru's personal prowess, Itachi is already familiar with all of his families Jutsu. Shikamaru would kneel down, put his hands together and stand back up and stab himself. That or he would try and run.
