And even now, he doesn't have legendary items, so he could definitely lose when it comes to Susano'o v Susano'o.

The idea that the two stalemate and Itachi runs out of stamina is a purely conservative one.

Because in the end, Itachi is the more intelligent and more skilled brother.

The Totsuka has only pierced humans. It has no feats in piercing Susanoo. So why do you assume it can?

It's real name is sword of kusanagi, which failed to pierce through 4tails Kyubi and it's even not as hyped as the original Kusanagi in terms of piercing power since the original Kusanagi is said to cut almost anything

Izanami, or a tricky genjutsu, or a sustained clone feint (like the one that caught Madara.)

Both Itachi and Sasuke can counter each other's jutsus but Sasuke can do so for much longer.

We don't know if Sasuke's Susano'o can block the Totsuka blade, which is hyped to be a flawless offense.

Wrong. It's called the Totsuka, and is a variant of the Kusanagi.

But that hardly means they're equal in all categories, because they're obviously quite different.

Not to mention that Totsuka's hype is > > > the Kusanagi.
