Hokages never let their guard down, even against their own. Hokage are always ready for any situation and must consider that anyone can possibly be a traitor, no matter how loyal they seem. Hiruzen as the Professor was certainly wise and observed everyone with the utmost caution.

Hiruzen must have judged that even with his back turned, Itach'si MS enhanced strength would be no match for him.

Hiruzen isn't stupid. He turned his back because he knows even if Itachi tried something funny then, Hiruzen would still stomp.
