Itachi had never technically dealt with a "healthy" Orochimaru.

The first time, Orochimaru was more then likely in a state similar to that of when he was against either Naruto or Sasuke. He did come with the intention to steal his body. Obviously he wasn't doing too well.

The second time, he had lacked his arms and was in a less then ideal position to fight back. His victories are somewhat hollow....
