Izanami was first activated with the first stab, but needs to be repeated in order to have the effect.....everything that happened when it was repeated was all real

But i can answer why the eyes changed a second time. It was because the loop set was complete. When using inzangi the opposite of inzanami, danzou or obito's eyes went shut after they completed the technique. Instead of deleting out of mid air like danzo, inzanami just repeats back to the beginning
where itachi clashed with kabuto. The second loop restarted at the beginning.....the clash

I see what you mean but inzanami was activated in the clash right? So why would inzanami
allow you to be vulnerable? My thought is that it repeats just like inzangi deletes in the intentional stage.
You don't just simply die. Why do you think itachi said it staeted when he first got stabbed.

Izanami has to record the intentionally(keyword) repeated pattern for it to be able to loop

once that pattern is manually repeated, as in having to do it for real and not in a genjutsu

only then will Izanami be complete and replay the actions over and over again

but if you get stabbed before closing the loop, you die.....Izanami is not Izanagi

U have to realize that at this particular scenario, both itach and even more so sasuke would defeat kabuto.
They were in freaking cave, spam amaterasu at sight, and engulf it in flames, kabuto woudnt survive
