Yeah he failed to change the system and way how the shinobi's lived killing each other for obvious reasons, so yeah he thought of changing that by becoming first uchiha hokage, yes you heard me right it was mentioned in one of his hiden and thanks to Konoha council that dream never come to pass and yeah he did thought like a hokage in his actions of massacre remember what hashirama told that he will kill anyone(madara) or even his family if they ever try to harm the village, that's what itachi did and all the consequences had occured as the after effect of his decisions. I think he never failed he did what was right and yes about sasuke he always has a plan and was always spoiled thanks to obito and Co. He entrusted naruto about sasuke and he only said so, so that naruto won't deviate from the path he has chosen Co-operation and team work.
