Itachi is a genjutsu specialist which makes him a deadly enemy.. but in Tsunade's defense, being a Senju, i think she knows how to deal with the sharingan in a way or two.. Ameterasu takes a while to work, and contrary to popular belief, Tsunade is not as slow as other people portray her to be.. if she manages to negate Itachi's genjutsu and ameterasu she has a strong change, but off course, Totsuka is another story..

Ummm... Itachi is not a Sannin. Tsunade is. I think she would be able to handle him, her chakra control and manipulation are top notch (they would have to seeing as she is a medical nin). It would be a boss match, and Itachi would probably win, but he wouldn't win easily. You saw the way he and Kisame fled from J-man, I doubt he would want to tango with the baddest bitch in Narutoverse.
