Sasuke vs Itachi was not a defeat, since Itachi planned it. Itachi is just the type of character that cannot lose to someone, he had to be freed from edo tensei, because the manga would end too fast if he were still there to beat madara

Death was not defeat since it was planned. Body double is not actual Itachi, Kabuto did not solo Itachi in any way, Itachi was always a step ahead of him, Obito never defeated Itachi in any way, he ran from Jiraya because he didnt want to kill him (he easily could have)
