Hard to say. Hiruzen was shown to do pretty well while under genjutsu just by using his other senses, however itachi's genjutsu can mirror the environment exactly with only enough difference in order to deliver a fatal blow. I don't know how hiruzen could get past susano'o, and i doubt he's fast enough to dodge amaterasu.

Just because he knows about Itachi's genjutsu dosen't mean he has an answer to it. What is doton going to do to the same Susanno that tanked Kirin? Amaterasu or Tsukuyomi is enough to easily beat Hiruzen.

Hiruzen is Edo. Amaterasu will burn him, but he will not feel pain so he can fight through it, or just touch Itachi with the burning part of his body. Unlimited Shadow clones for Hiruzen means he wins. Itachi will not know who to Amaterasu or use genjutsu on. Good Game.
