But I'd say he didn't had AIDS, because coughing blood is not necessarily a symptom of AIDS. AIDS is just a disease that weakens the immune system, which ultimately allows germs to enter and attack body easily.

But Itachi's internal body temperature was so hot that germs couldn't have survived in his body anyway, because he could spit out fire from stomach... you can imagine the heat burning up in the body... there's no way any disease causing germ could have survived that heat.

I personally think he had cancer of some kind. But people will still make jokes out of that and say "he had penile cancer" or something. You can't stop it
