It's staged. It's categorized as TV Shows and just like reality shows, serials, soaps and other fictions works, it's scripted and practiced. It's not a sport.
They rehearse for those matches and the winner is decided before the match. It's for entertainment and I watch it for that. I mean, people watch movies, serials ect... and this is acting+performance (basically it's fiction) and saying that you're not watching because it's fake, is just a ridiculous statement. It's like saying you don't see movies because it's fake.
Either you like it for entertainment purposes or dislike it because it doesn't appeal to you. It's as simple as that.
UFC is pretty more real though some parts are also staged. Bit it's nearly as boring as boxing is. Boxing is real.
The fact is: Sports are real, TV Shows are fictions works. But that doesn't change the fact that you can enjoy both.
As for your question on the 3 parts.
Injuries happen due to bad luck and bad timing. They trained and practiced for it but just obviously, once they are wrestling, in front of the crowd, there are chances that things go wrong.
Long ago, they used to self-injure themselves for more drama (See "The Wrestler" movie) but at times, it were real due to mis-timing just like for injuries. Recently, they stopped inflicting self-injuries and in case of real blood-cuts, medics are rushed for help. They also banned chair head shots for that reason.
Slams and all moves in a match are practiced/rehearsed (together and individually).
