Maybe it's old but since Uchiha kills their best friends to obtain the MS and brothers, parents etc for ems why Itachi don't had it already? Furthermore he didn't kill Shisui but he had the MS before he slaughtered his clan? Plot hole?

Shisui was Itachi's best friend, and the pain and etc from his death awakened Itachi's MS. You don't have to kill your best friend to obtain MS, but have to go through the trauma or something like that.

You don't get EMS by killing your best friend, you get it by taking the eyes of a close family member who has also awakened the MS. MS isn't necessarily activated by killing your best friend, but by dealing with the emotional grief of losing the person you were closest to. Itachi was there when Shisui committed suicide. He watched his best friend die.

I don't think that you must Kill a friend to have MS, the sadness and rage for someone you love will awake it (remember that sasuke awakened MS after knowing the truth of Itachi.
