it was kinda mentioned indirectly in the manga i think oro said some place (dont remember the name) was almost as famous the shikkotsu forest which is where the slugs reside just like how the toads live at mt myboku and the snakes live at ryuchi cave and they both have sage modes so there probably is a slug sage mode

Actually the reason could be that she can't heal others while in it. Thus it could only be used when she fights alone or something?
I mean, apparently yin seal chakra can't be used to heal people since Tsunade and Sakura are out of chakra and still have their yin seals intact. This makes me wonder if the chakra in the seal is actually senjutsu chakra that will petrify others if used directly on someone. Katsuyu could work as a catalyst here.

Possibly. Nothing proving it exist, but it's well hinted at.

For me I see it as the three Sannin has their animals. Jiraiya has Toads, Orochimaru has Snakes, and Tsunade has Slugs. We've seen sage modes for Toads and Snakes (well, dragons, but same difference in this case), so personally I see it logically that there is a chance for a slug mode. So yeah, not solid evidence but it's not an unlikely thing to happen. We've seen more crazy events.
