Like it's been said, he just doesn't know how to express himself properly.
Instead of indirectly insulting someone, he just goes and tries to hit with a low blow i.e criticizing Naruto's penis size.
As for his sexual preference?
I would go on a limb and say he probably has no real preference.
He doesn't know the feeling of love or being infatuated with someone so logically, he probably doesn't like men or women.
