Whether it is reincarnation or straight immortality Sasuke has options. Ppl forget Sasuke has been learning from Orochimaru who has been seeking what Sasuke has in order to attain immortality.

So Sakura can be forever young and Sasuke can be immortal.

I think I read that being the Juubi Jin took a toll on his body and that led to his death. Also, I remember reading that those with rinnegan exist outside life and death
