Is Sakura the only one Naruto uses honorifics for?

Actually, Naruto wasn't even friends with Sakura when he started calling her Sakura-chan. He did so because he WANTED to hook up with her, and even he knew that barging in as a creepy stalker without respecting her would weaken his prospects.

As for others, he didn't feel the need to show that respect, because most others weren't pushing him away the way Sakura was. He found it easier to bond with others, whereas Sakura was pushing him away even as far as the end of part one, and although she DID think of removing him from the friendzone at the start of part 2, she quickly changed her mind.

Naruto respects his teachers (mostly of them, anyway), and probably used honorifics when he spoke to his parents too (this probably got lost in translation --- the reason why this issue so glaring). For friends, he often felt closer to them before actually making acquaintances, so wouldn't have felt the need to use honorifics.

If I'm not mistaken I think Sakura was the only one and that was because she was his crush.

Because she is not that close to him. Inspire of being a friend he still calls her chan and other calls other people by name or nicknames means that he doesn't still feel that Sakura is close to him. Be realises how superficial Sakura is an therefore gives her the superficial treatment

He used that honorific because he liked her in the past and wanted to gain her respect as well as getting closer and closer to her.

Some people use chan to call things they find cute

Naruto does find sakura cute till this day therefore Sakura chan(plus its a habit of many years)
