Shikamara Far BETTER. Shikamara true enough doesn't any real wide ranging talent, but he is a master tactican n genius. Shikamara is better actually off the field and as a General. Sakura on the other hand besides her talent for chakra controll n excellent medicine practices is a weaker choose. SIMPLY... if you where a soldier and got into a heavy combat situation n hit.... a Great Doctor Could Save Your Life But A Great General Wouldn't Have Put You In The Situation To Begain With.

If you are refering to who is overall more useful then it's shika, Sakura is capable of saving many of the battlefield but Shikamaru can keep them from getting hurt in the first place.

However 1v1 Shika without jutsus he won't stand a chance unless he gets to prepare traps in beforehand or something. He is not quicker and he is certainly not stronger. His only chance is knocking her out with a direct hit kunai (unlikely) or trying to set something up with explosive tags (even more unlikely to work unless he has time to prepare beforehand.). What people seem to forget is that Shika is an incredible tactician sure but no one can win a game of chess without any pieces. That is the same reason why part 1 Shika often struggled even though he was intellectually superior. He's severely lacking in firepower in comparrison with others, he had to forfeit against Temari, he was losing to the sound 4 girl untill Temari arrived. He lacked firepower and the same would happen in a fight with Sakura if he didn't have any jutsus.
