Marrying someone who shares the same surname with you and blood is different from marrying a person who shares the same skin colour as you and almost definitely has such a far back biological relationship with you it wouldn't matter.

Itachi wasn't a healthy child and I'd argue Sasuke is pretty stupid.
But anyway, opinions.
It's a manga, and real life consequences from inbreeding don't apply according to Kishi but that doesn't make it less wrong. It's still a disturbing concept to me, fiction or not.
