Photoshop IS hard to use when you have absolutely no idea how all of the tools work and what all of them can do.

photoshop is quite easy to use, but if you are designing what you are putting on the shirts or doing any graphic design at all you might want to use illustrator, the interface is pretty much the same as photoshop. the difference is that photoshop, while i use it for graphic design, is made for photo editing, illustrator is full on a graphic design tool.

If you're doing designs that will be scaled differently you definitely want to use illustrator, as photoshop works with bitmaps whilst illustrator can create vectors.

Depends. If you're trying to do something simple, its easy. But if you're trying to do some complicated shit, its obviously going to be complicated

It's pretty easy to do.
I would suggest using either Photoshop, Sai, or Manga Studio, and draw using a Wacom tablet, these are usually the best and are mostly cheap.

The rest depends on your drawing skills.
If you're not that great, I advice you to use Manga Studio to draw your line art, and use Photoshop for coloring. Manga Studio automatically adjusts your lines when you draw them, making it more fluid-, and natural looking.
