It is very possible danzou snuck into konoha, after the transplant and faked his death.
Maybe madara needs and wants someone on the inside, and maybe, just maybe, madara has planned itachis and sasukes future entirely, afterall, madara said it was fate and destiny that gave itachis life its meaning.
These two brothers (madara and bro) could have waited for the next set of exceptional brothers, and danzou (madaras brother) forced itachi into a life of a spy, and to kill the uchiha (afterall, madara hates uchiha for betraying them, and logically-so does the brother) and as thus they knew how itachi would save sasuke.
I just find it strange that madaras actions suggest sasuke is a BIG part of his plans, otherwise he wouldnt try so hard.
And actually, this theory fits nicely, maybe sasukes and itachis life was planned by madara and danzou, as they appear to have a lot of connections to there life
